    Aconiti Decoction is widely utilized in orthopedic practice, yet current research faces the following challenges:(1) Toxicity and Clinical Safety: The diester-diterpenoid alkaloids in Aconiti Decoction, such as aconitine, mesaconitine, and hypaconitine, exhibit significant pharmacological activity but also pose toxicity risks.Their therapeutic and toxic doses are closely aligned, resulting in a narrow therapeutic window and a high risk of adverse reactions or poisoning, so ensuring safe and effective clinical application remains a critical issue.(2) Mechanistic Exploration: Although Aconiti Decoction demonstrates efficacy in alleviating inflammation, swelling, and pain in rheumatoid arthritis, its mechanisms remain unclear.Studies suggest it modulates inflammatory cytokines, reduces levels of RA-associated autoimmune antibodies, and balances T-cell subsets.Future research should focus on its anti-inflammatory, analgesic, immunomodulatory, and anti-angiogenic effects, particularly its regulation of inflammatory networks and specific signaling pathways.(3) Optimization of Treatment Regimens: The correct formulation and decoction methods can reduce toxicity and enhance therapeutic effects.Future research should focus on optimizing treatment plans, including drug ratios, decoction methods, and dosages, so as to improve treatment outcomes and reduce drug toxicity.(4) Establishment of Quality Control and Evaluation Standards: By integrating traditional experiences with modern analytical techniques, more scientific and reasonable quality control and evaluation standards should be established to promote the modern research of aconite-based Chinese medicinals and ensure their more rational and scientific applications in clinical practice.In conclusion, future research should focus on the safety assessment of Aconite Decoction, in-depth exploration of its mechanism of action, optimization of treatment regimens, improvement of therapeutic effects, and reduction of toxicity risks.At the same time, a scientific quality control and evaluation system should be established to ensure the safe and effective applications of Aconite Decoction in clinical practice.

    The comprehensive analysis of the four diagnostic methods in Huangdi′s Internal Classic adheres to the holistic view of heaven, earth, and human beings, utilizing harmony between humans and nature, as well as the internal equilibrium of the human body, as criteria for comprehensively and accurately diagnosing diseases.This approach achieves dialectical unity between subjectivity and objectivity, phenomena and essence, and the whole with its parts.Inspection prioritizes observing vitality, followed by complexion analysis and comprehensive visual assessment.It correlates internal visceral functions with external manifestations, deducing pathological patterns through the interplay of "harmony versus disharmony" and "correspondence versus non?correspondence." Auscultation and Olfaction are grounded in the five-element theory, elaborating on the relationships among the five zang organs, five tones, five voices, and odors to discern disease characteristics and progression laws.Inquiry aims to identify the core pathogenesis, establish precise disease nomenclature or syndrome differentiation, and provide a foundation for treatment.Palpation emphasizes four principles: pulse correspondence with seasonal variations, pulse vitality rooted in stomach qi,integrative analysis of pulse and complexion, and "respiratory synchronization during pulse-taking".Clinicians must synthesize information from the above four methods to guide the transformation from "disharmony" to "harmony."

  • Study on Traditional Chinese Medicine Treatment for Chronic Pelvic Pain Caused by Adenomyosis Based on Macrophage Polarization

    The therapeutic mechanism of TCM in treating adenomyosis is associated with immunomodulation and improvement of the inflammatory microenvironment.Macrophages, as one kind of key immune cells involved in inflammatory responses, undergo phenotype transformation(that is, macrophage polarization) under environmental stimuli.Dysregulated macrophage polarization is closely linked to the pathogenesis of chronic pelvic pain(CPP) secondary to adenomyosis.According to the related TCM theory, blood stasis in the uterine chamber is the fundamental pathogenesis of adenomyosis, while combined dampness-stasis serves as the core mechanism underlying pain manifestation.Inflammatory cytokines produced through macrophage polarization exhibit significant correlations with TCM blood stasis syndrome.The method of clearing dampness and resolving blood stasis alleviates pain by modulating macrophage polarization-related secretory factors and signaling pathways, thereby improving hypoxic conditions, delaying fibrosis, and suppressing algogenic factor expression to impede disease progression.Besides, factors such as vascular endothelial growth factor(VEGF),estrogen, and algogenic mediators, etc.are intricately involved in macrophage polarization.Whether the pain-relieving effects of the above factors are achieved by interfering with macrophage polarization is a direction worth further exploration in the treatment of CPP caused by adenomyosis with the method of clearing dampness and removing blood stasis in the future.

  • Academic Ideology of "Family Yin′s Qi-Regulating and Stagnation-Resolving Therapy(School)" Based on the Theory of "Qi Monism"

    Family Yin′s TCM School was originated during the Jiaqing period of the Qing Dynasty(1796-1820),and has been inherited through eight generations over two centuries.Rooted in the "Qi Monism" theory, it has developed into the distinctive "Family Yin′s Qi-Regulating and Stagnation-Resolving Therapy(School)".This school believes that the occurrence of diseases is attributed to the imbalance of the essence of qi,such as qi deficiency and qi excess(heat),as well as the dysfunction of qi transformation and qi movement. "Regulating qi" means "adjusting qi".Here, "qi" has three layers of meanings: the first is the essence of qi;the second is qi transformation, that is, the generation and change of qi;the third is qi movement, that is, the ascending, descending, exiting, entering, gathering and dispersing of qi.Any abnormality in the above three will lead to the occurrence of diseases.The meaning of "regulating qi" has four aspects: first, replenishing the deficiency of qi;second, clearing the excess of qi;third, facilitating the transformation of qi;and fourth, resolving the stagnation of qi.The "stagnation" in "relieving stagnation" also has two layers of meaning: one is the "six stagnations",namely qi,blood, phlegm, dampness, food retention, and fire; the other refers to the "stagnation of qi movement and transformation". "Relieving stagnation" can either mean "eliminating the six stagnations" or "facilitating the movement and transformation of qi".In clinical practice, the first priority is to address the root cause and fundamental issue.For instance, when the "qi" of the internal organs and the "six stagnations" interact as cause and effect, leading to various diseases, treatment should not only adjust the abnormal functions of the qi of the internal organs and its transformation and movement, but also attach importance to resolving the "six stagnations".At the same time, it is also necessary to take into account the urgency of symptoms and consider the treatment of both the cause and symptoms.The combination of "regulating qi" and "resolving stagnation",with flexible and adaptable application, can solve many clinical problems.

  • Study on the Historical Records of Virgate Wormwood Decoction

    Objective: To analyze the origin, drug composition, formula rationale, functions and indications, dosage, drug processing, decocting method and administration method of Virgate Wormwood Decoction, providing a literature basis for the research and development of this formula.Method: Based on the review of relevant books and documents, the key information of Virgate Wormwood Decoction was comprehensively verified by using the bibliometric method.Results:(1) Virgate Wormwood Decoction was first recorded in Treatise on Cold Damage Diseases by Zhang Zhongjing of the Eastern Han Dynasty.It is composed of three herbs: Yinchenhao(Herba Artemisiae Scopariae),Zhizi(Fructus Gardeniae) and Dahuang(Radix et Rhizoma Rhei).Historically, it has been known by various alternative names such as Yinchen Decoction, Diretang Decoction, Da Yinchen Decoction, and Yinchen Dahuang Decoction;(2) The composition of its ingredients, the decocting method, and the way of administration have been widely adopted by later generations of physicians, and there is little controversy over its therapeutic rationale;(3) In the past, physicians used it to treat conditions where pathogenic heat invades the interior and dampness-heat obstructs the middle energizer;(4) The dosage of Virgate Wormwood Decoction can be converted to: 82.5 g of Yinchenhao, 14 g of Zhizi, and 27.5 g of Dahuang;(5) Most physicians follow Zhang Zhongjing′s processing methods, namely, splitting Zhizi and peeling off the skin of Dahuang, which is consistent with the records in the 2020 edition of The Pharmacopoeia of the People′s Republic of China.Conclusion: Verifying the information such as the composition of herbs, functions and indications, and dosage of Virgate Wormwood Decoction can provide a basis for research and application of this formula.

  • Study on the Molecular Mechanism of Huanglian in Treating Colorectal Cancer Based on WGCNA

    Objective: To study the molecular mechanism of Huanglian(Rhizoma Coptidis) in treating colorectal cancer using weighted gene co-expression network analysis(WGCNA) and other bioinformatics techniques.Methods: The chemical components and related targets of Huanglian were searched through the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform(TCMSP,http://tcmspw.com/TCMSP).The Swiss Target Prediction platform was used to obtain targets related to the chemical components of Huanglian.Cytoscape was employed to construct a "Huanglian-compound-target" network, filtering for active compounds.Differentially expressed genes(DEGs) between colorectal cancer and normal samples were retrieved from the UCSC database, and WGCNA was used for differential analysis.The Cancer Genome Atlas(TCGA) database was used to obtain DEGs.The intersection of active compound-related targets, WGCNA-derived DEGs, and TCGA-derived genes was analyzed.Protein interaction and enrichment analysis of the intersecting targets were conducted using the STRING database.Univariate Cox regression and LASSO algorithms were applied to optimize the model, followed by prognosis analysis.The hub gene mutation status was analyzed using the CBioPortal database.Immune infiltration was analyzed using the CIBERSORT algorithm, while hub gene expression in immune cells was evaluated using the TIMER algorithm.Molecular docking was validated using Autodock software.Results: A total of 4 798 DEGs were identified from tumor and normal samples.WGCNA analysis of these genes yielded 1 280 significantly different genes.The TCMSP and Swiss Target Prediction databases identified 11 active compounds of Huanglian and 333 related targets.The "Huanglian-active compound-target" network analysis revealed magnolol, feruloyl tyramine, berberine, and quercetin as the main active components.A total of 41 intersecting genes were identified by combining active compound-related targets with WGCNA and TCGA results.Univariate Cox analysis identified eight genes significantly associated with prognosis.After model optimization through external dataset validation, Cox regression, and the LASSO algorithm, four hub genes were identified.Immune infiltration analysis revealed a negative correlation between resting mast cells and activated mast cells, and a positive correlation between M2 macrophages and resting mast cells.The hub genes significantly influenced immune cell infiltration.Molecular docking showed strong binding activity between the active components and the targets.Conclusion: Huanglian may exert its therapeutic effect on colorectal cancer by targeting genes such as CA2,CHEK1,TOP2A,and TRAP1 through active components like magnolol, feruloyl tyramine, berberine, and quercetin, etc.

  • Study on the Correlation Between Rheumatoid Arthritis of Liver-Kidney Yin Deficiency Type Combined with Hashimoto′s Thyroiditis and Rheumatoid Arthritis

    Objective: To observe the correlation between rheumatoid arthritis of liver-kidney yin deficiency type combined with Hashimoto′s thyroiditis and rheumatoid arthritis(RA).Methods: A total of 51 patients with RA of liver-kidney yin deficiency type admitted to Guanghua Hospital of Integrated Traditional Chinese and Western Medicine in Shanghai from August 1,2023 to February 1,2024 were selected.If the patients had thyroid peroxidase antibody(TPO-Ab) > 34.0 IU·mL~(-1) and/or thyroglobulin antibodies(TGAb) >115.00 IU·mL~(-1),they were included in the RA combined with Hashimoto′s thyroiditis antibody positive(RA-HT) group; otherwise, they were included in the RA group.Ten healthy subjects were selected as the healthy control group.General data, TCM symptom score sheets, health assessment questionnaire(HAQ) scores, and the ratio of peripheral helper T cells(Tph) in peripheral blood of the RA-HT and RA groups were compared, and the ratio of Tph in peripheral blood of the three groups was detected.Results: There were no statistically significant differences in age, disease duration, and body mass index(BMI) between the RA-HT group and the RA group(P>0.05).The RA-HT group had significantly higher scores in TCM symptom assessment, HAQ,IgM,TPOAb, TGAb, and TNF-α levels compared with the RA group, with statistically significant differences(P<0.05).Compared with the RA group and the normal group, the Tph level in the RA-HT group was significantly increased, with a statistically significant difference(P<0.05).Conclusion: The level of inflammation in the body of RA-HT patients is higher, and the increase in Tph ratio may be one of the reasons for the positive thyroid autoantibodies in RA patients.

    Objective: To observe the clinical efficacy of Qi-Supplementing and Kidney-Tonifying and Stasis-Removing Decoction on recurrent spontaneous abortion(RSA) with positive anti-cardiolipin antibody(ACA) in kidney deficiency and blood stasis type.Methods: A total of 96 patients with ACA-positive RSA of kidney deficiency and blood stasis type admitted to Zhoukou Hospital of Traditional Chinese Medicine from January 2020 to June 2023 were selected and randomly divided into the control group and the observation group, with 48 cases in each group.The control group was treated with Enteric-Coated Aspirin Tablets combined with Prednisone Acetate Tablets, while the observation group was treated with Qi-Supplementing and Kidney-Tonifying and Stasis-Removing Decoction on the basis of the control group′s treatment.The ACA-IgG titer, ACA-IgM titer, ACA-IgA titer, TCM syndrome scores, clinical efficacy and safety were observed in both groups.Results: In terms of the therapeutic effect of TCM syndromes, the effective rate of the observation group was 93.75%,while that of the control group was 79.17%.The effective rate of the observation group was higher than that of the control group, and the difference was statistically significant(P<0.05).After 1,2,and 3 courses of treatment, the TCM syndrome scores of the observation group were lower than those of the control group, but the differences were not statistically significant(P<0.05).In terms of the therapeutic effect of ACA-IgG,the effective rate of the observation group was 95.83%,while that of the control group was 83.33%.The effective rate of the observation group was higher than that of the control group, and the difference was statistically significant(P<0.05).After three courses of treatment, the titer values of ACA-IgG,ACA-IgM,and ACA-IgA in the observation group were lower than those in the control group, and the differences were statistically significant(P<0.05).The total incidence of adverse reactions in the observation group was 2.08%,while that in the control group was 14.58%.The incidence in the observation group was lower than that in the control group, and the difference was statistically significant(P<0.05).Conclusion: Qi-Supplementing and Kidney-Tonifying and Stasis-Removing Decoction can significantly improve the clinical efficacy in the treatment of RSA with positive ACA in kidney deficiency and blood stasis type, and effectively regulate the immune function of patients.

  • Clinical Research on Modified Wind-Dispersing and Nodules-Dissipating Decoction in the Treatment of Adenoid Hypertrophy in Children

    Objective: To observe the clinical efficacy of Modified Wind-Dispersing and Nodules-Dissipating Decoction on adenoid hypertrophy in children under the guidance of the theory of latent pathogenic factors.Methods: A total of 154 children who underwent treatment for adenoid hypertrophy in our hospital from January 2022 to December 2023 were randomly divided into the study group and the control group.The children in the control group were treated with conventional Western medicine, while those in the study group were treated with Modified Wind-Dispersing and Nodules-Dissipating Decoction.The clinical efficacy, changes in TCM syndrome scores, serum indicators, and quality of life before and after treatment were compared between the two groups.Results: The effective rate of the study group was 93.51%,while that of the control group was 74.03%.The effective rate of the study group was higher than that of the control group, and the difference was statistically significant(P<0.05).After treatment, the scores of nasal congestion and snoring, mouth breathing, chest tightness and shortness of breath, repeated breathing pauses and drowsiness of the children in the study group were all lower than those of the control group, and the differences were statistically significant(P<0.05);the levels of SIgE,sIL-2R,TNF-α and IL-4 of the children in the study group were all lower than those of the control group, and the differences were statistically significant(P<0.05);the quality of life scores of the children in the study group were higher than those of the control group, and the difference was statistically significant(P<0.05).Conclusion: Under the guidance of the theory of latent pathogenic factors, Modified Wind-Dispersing and Nodules-Dissipating Decoction is effective in treating adenoid hypertrophy in children, can significantly improve the therapeutic outcome, reduce the TCM syndrome scores and serum indicators, and enhancing their quality of life.

  • The Influence of Diversified Local Acupoint Stimulation Combined with Conventional Treatment on TCM Syndrome Scores and Gastrointestinal-Related Indicators in Patients with Atrophic Gastritis

    Objective: To observe the influence of diversified local acupoint stimulation combined with conventional treatment on TCM syndrome scores and gastrointestinal-related indicators in patients with atrophic gastritis.Methods: A total of 79 patients with atrophic gastritis who were treated at The First Affiliated Hospital of Henan University of Chinese Medicine from January 2022 to December 2023 were selected and randomly divided into the control group with 39 cases and the observation group with 40 cases according to the random number table method.The control group was given conventional Western medicine treatment, while the observation group was treated with diversified local acupoint stimulation on the basis of the control group′s treatment.The TCM syndrome scores, inflammatory factors, gastrointestinal hormones, gastroscopy indicators and clinical efficacy of the two groups were observed.Results: After treatment, the TCM syndrome scores of the observation group were lower than those of the control group, and the differences were statistically significant(P<0.05);the inflammatory indicators of the two groups were lower than those before treatment, and the observation group′s were lower than those of the control group, with statistically significant differences(P<0.05);the gastrin levels of the two groups were lower than those before treatment, while the somatostatin and motilin levels were higher than those before treatment.Moreover, the gastrin level of the observation group was lower than that of the control group, while the somatostatin and motilin levels were higher than those of the control group, with statistically significant differences(P<0.05).The gastroscopy indicators of the two groups were lower than those before treatment, and the observation group′s were lower than those of the control group, with statistically significant differences(P<0.05).The effective rate of the observation group was 95.00%,while that of the control group was 79.49%.The effective rate of the observation group was higher than that of the control group, with a statistically significant difference(P<0.05).Conclusion: Diversified local acupoint stimulation combined with conventional treatment can effectively improve the clinical symptoms, inflammatory factors and gastrointestinal function of patients with atrophic gastritis.

  • Clinical Observation of Modified Center-Supplementing and Qi-Boosting Decoction Combined with Transurethral Plasma Kinetic Enucleation of Prostate in the Treatment of Benign Prostatic Hyperplasia Complicated with Detrusor Underactivity

    Objective: To observe the clinical efficacy of Modified Center-Supplementing and Qi-Boosting Decoction combined with transurethral plasma kinetic enucleation of prostate(PKEP) in the treatment of benign prostatic hyperplasia(BPH) complicated with detrusor underactivity(DU).Methods: A total of 68 patients with BPH combined with DU who underwent PKEP surgery in Fangcun Hospital of Guangdong Provincial Hospital of Chinese Medicine from December 2022 to December 2023 were selected and randomly divided into the control group with 35 cases and the treatment group with 33 cases according to the random number table method.All patients underwent PKEP surgery, and both groups received conventional treatment regimens after surgery.The treatment group was additionally treated with Modified Center-Supplementing and Qi-Boosting Decoction on the basis of conventional treatment.The clinical efficacy, perioperative complications, and changes in IPSS score, IPSS-V score, IPSS-S score, QoL score, post-void residual urine volume(PVR),and maximum urine flow rate(Qmax) before and after treatment were observed in both groups.Results: There were no statistically significant differences between the two groups in terms of the weight of the excised tissue during the operation, the decrease in sodium ions and hemoglobin after the operation, blood loss, hospital stay, and operation time(P>0.05).One month and three months after the operation, the IPSS score, IPSS-V score, IPSS-S score, QoL,PVR and Qmax of both groups were all better than those before the treatment(P<0.05).There were no statistically significant differences in the IPSS score, IPSS-V score, IPSS-S score, QoL and PVR between the two groups one month and three months after the operation(P>0.05).The Qmax of the observation group was lower than that of the control group one month and three months after the operation, and the difference was statistically significant(P<0.05).The effective rate of the treatment group was 78.79%,and that of the control group was 82.86%.There was no statistically significant difference in the effective rate between the two groups(P>0.05).Conclusion: Modified Center-Supplementing and Qi-Boosting Decoction combined with PKEP surgery in the treatment of BPH complicated with DU can improve the symptoms and quality of life of patients and increase their postoperative Qmax.

  • An Analysis of TCM Constitution Types of Patients with Gestational Diabetes Mellitus and Their Correlation with Pregnancy Outcomes

    Objective: To analyze the TCM constitution types of patients with gestational diabetes mellitus(GDM) and their correlation with pregnancy outcomes.Methods: A retrospective study was conducted on 160 pregnant women who underwent regular prenatal check-ups at Ganzhou Maternity and Child Health Hospital from June 2023 to June 2024.They were divided into the GDM group(51 cases) and the control group(109 cases without pregnancy complications).Multivariate Logistic regression was used to analyze the correlation between TCM constitutions of GDM patients and adverse pregnancy outcomes.Results: The incidence of adverse pregnancy outcomes in the GDM group was 31.37%,which was higher than 6.42% in the control group, and the difference was statistically significant(P<0.05).There were statistically significant differences in age, family history of diabetes, history of polycystic ovary syndrome, and TCM constitution types between the two groups(P<0.05).The regression coefficients of advanced maternal age, family history of diabetes, history of polycystic ovary syndrome, preference for sweet foods in diet and phlegm-dampness constitution were 0.220,1.727,1.952,0.236 and 4.107 respectively, which were positively correlated with adverse pregnancy outcomes in GDM patients(P<0.05);the risk of adverse pregnancy outcomes in GDM patients with phlegm-dampness constitution was 60.79 times greater than that of individuals with a balanced constitution.Conclusion: Advanced maternal age, family history of diabetes, history of polycystic ovary syndrome, preference for sweet foods in diet and phlegm-dampness constitution are independent risk factors for adverse pregnancy outcomes in pregnant women.The risk of GDM in the phlegm-dampness constitution population is significantly higher than that in other constitution types.

  • Clinical Research on Huanglian Hypoglycemic Formula in the Treatment of Type 2 Diabetes Mellitus with Syndrome of Excessive Heat Damaging Fluid

    Objective: To observe the clinical efficacy of Huanglian Hypoglycemic Formula on type 2 diabetes mellitus with syndrome of excessive heat damaging fluid.Methods: A total of 80 patients with diabetes mellitus having syndrome of heat excess damaging fluid who were treated at Wan Cheng Qu Hospital of TCM in Nanyang City from March 2021 to March 2022 were selected and randomly divided into the control group and the treatment group, with 40 cases in each group.Both groups were given basic treatment.The control group was then additionally treated with metformin, while the treatment group was further treated with Huanglian Hypoglycemic Formula on the basis of the control group.Both groups were treated continuously for 8 weeks.The therapeutic effects and drug safety of the two groups were evaluated, and the differences in TCM syndrome scores, glucose metabolism, blood lipids, insulin, serum indicators, and renal function indicators before treatment and 8 weeks after treatment were compared between the two groups.Results: The effective rate of the control group was 67.50%,while that of the treatment group was 87.50%.The difference between the two groups was statistically significant(P<0.05).The TCM syndrome scores in both groups were significantly lower after treatment than those before treatment, and the treatment group had lower scores than those of the control group(P<0.05).After treatment, fasting plasma glucose(FPG),2-hour postprandial plasma glucose(2hPG),glycosylated hemoglobin(HbA1c),low density lipoprotein cholesterol(LDL-C),triacyl glycerol(TG),total cholesterol(TC),insulin resistance index(HOMA-IR),retinol-binding protein(RBP4),fatty acid-binding protein(FABP),serum creatinine(Scr),blood urea nitrogen(BUN),and urinary albumin excretion rates(UAER) were significantly reduced in both groups, and the treatment group′s above indicators were lower than those of the control group(P<0.05).High density lipoprotein cholesterol(HDL-C),β-cell function(HOMA-β),and β-arrestin2 were significantly increased, and the treatment group′s above indicators were higher than those of the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion: Huanglian Hypoglycemic Formula can improve the symptoms of patients with type 2 diabetes mellitus having syndrome of excessive heat damaging fluid, regulate blood glucose and lipid levels, improve insulin resistance, and has no obvious adverse reactions.

  • Clinical Study on the Treatment of Oral Phase Dysphagia after Stroke with Tongue Acupuncture and Facial Acupuncture Combined with Swallowing Rehabilitation Therapy

    Objective: To observe the clinical efficacy of tongue acupuncture and facial acupuncture combined with swallowing rehabilitation therapy on oral phase dysphagia after stroke.Methods: A total of 60 patients with oral phase dysphagia after stroke treated at The First Rehabilitation Hospital of Shanghai from July 2021 to July 2023 were selected and randomly divided into the control group with 28 cases and the observation group with 32 cases by the coin-tossing method.The control group was treated with swallowing rehabilitation therapy, while the observation group was treated with tongue acupuncture and facial acupuncture on the basis of the control group′s treatment.The videofluoroscopic swallowing study(VFSS),oral feeding ability, Watian′s water-drinking test rating, tongue muscle strength and clinical efficacy of the two groups were compared before and after treatment.Results: After treatment, the VFSS scores of both groups were higher than those before treatment(P<0.05),and the VFSS score of the observation group was higher than that of the control group, with a statistically significant difference(P<0.05).The oral transport time, pharyngeal transport time, and pharyngeal delay time of both groups were lower than those before treatment(P<0.05),and the oral transport time, pharyngeal transport time, and pharyngeal delay time of the observation group were lower than those of the control group, with statistically significant differences(P<0.05).The Watian′s water-drinking test ratings of both groups were better than those before treatment(P<0.05),and the Watian′s water-drinking test rating of the observation group was better than that of the control group, with a statistically significant difference(P<0.05).The average tongue pressure, tongue pressure duration, and tongue pressure peak of the observation group were higher than those of the control group, with statistically significant differences(P<0.05).The effective rate of the observation group was 90.62%,and that of the control group was 71.43%.The effective rate of the observation group was higher than that of the control group, with a statistically significant difference(P<0.05).Conclusion: The combination of tongue acupuncture and facial acupuncture with swallowing rehabilitation therapy can improve the abnormal swallowing function and tongue muscle strength of patients with oral phase dysphagia after stroke, and promote the recovery of oral feeding ability.

  • The Effect of Acupoint Needling Combined with Three Fu Patch on Serum Bone Metabolism and Lequesne Index in Senile Patients with Knee Osteoarthritis

    Objective: To observe the clinical efficacy of acupoint needling combined with three fu patch therapy in treating knee osteoarthritis(KOA) in the elderly, as well as its effect on serum bone metabolism and Lequesne index.Methods: A total of 100 patients with KOA treated at The Second Affiliated Hospital of Jiangxi University of Chinese Medicine from July 2022 to July 2023 were selected and randomly divided into the control group and the combined group, with 50 cases in each group.Both groups received basic rehabilitation training.The control group was additionally treated with three fu patch application, while the combined group received acupuncture at acupoints in addition to the treatment given to the control group.The clinical efficacy, incidence of adverse reactions, TCM syndrome scores, Lysholm functional scores, Lequesne index of the two groups were observed.The levels of serum bone glaprotein(BGP),osteoprotegerin(OPG),C-telopeptide of type Ⅰ collagen(CTX-Ⅰ),interleukin(IL)-1,matrix metalloproteinase(MMP)-3,and tumor necrosis factor(TNF)-α before and after treatment were detected and compared.Results: After treatment, the TCM syndrome scores, Lequesne index and Lysholm function scores of both groups of KOA patients were all better than those before treatment(P<0.05).Moreover, the TCM syndrome scores and Lequesne index of the combined group were lower than those of the control group after treatment, while the Lysholm function score was higher than that of the control group, with statistically significant differences(P<0.05).The levels of BGP and OPG in the combined group were higher than those in the control group, while the level of CTX-Ⅰ was lower than that in the control group, with statistically significant differences(P<0.05).The levels of IL-1,MMP-3 and TNF-α in the combined group were lower than those in the control group, with statistically significant differences(P<0.05).The effective rate of the combined group was 92.00%,while that of the control group was 76.00%.The effective rate of the combined group was higher than that of the control group, with a statistically significant difference(P<0.05).The incidence of adverse reactions in the control group was 8%,and that in the combined group was 4%.There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion: Acupoint needling combined with three fu patch in the treatment of elderly patients with KOA can regulate the levels of serum inflammatory factors and bone metabolism indicators, reduce the Lequesne index, improve the bone and joint function, enhance their quality of life, and has a low incidence of adverse reactions.

  • Clinical Research on Thumbtack Needle Combined with Blood Mansion Stasis-Expelling Decoction for Treating Cardiac Dysfunction in Ischemic Cardiomyopathy

    Objective: To evaluate the clinical efficacy of thumbtack needle combined with Blood Mansion Stasis-Expelling Decoction on cardiac dysfunction in ischemic cardiomyopathy.Methods: A total of 124 patients with cardiac dysfunction in ischemic cardiomyopathy admitted to First Affiliated Hospital of Henan University of Chinese Medicine from July 2023 to August 2024 were randomized into the study group and the control group using the random number table, with 62 cases in each group.The control group received conventional therapy, while the study group received additional thumbtack needle therapy combined with Blood Mansion Stasis-Expelling Decoction.Clinical efficacy, and the changes in TCM syndrome scores, cardiac function, biochemical indicators and heart failure severity before and after treatment were compared between the two groups.Results: The study group exhibited a significantly higher total efficacy rate(P<0.05).After treatment, TCM syndrome scores in both groups decreased than those before treatment, Post-treatment for chest pain(2.03±0.21),palpitations(1.26±0.28),chest tightness(1.35±0.31),shortness of breath(1.52±0.26),and fatigue(1.42±0.28) were significantly lower than those of the control group(P<0.05).After treatment, the levels of LVEF and SV in both groups were higher than those before treatment, while the levels of LVESD,LVEDD,and LVMI were significantly lower than those before treatment.The differences between the groups were statistically significant(P<0.05).The NT-proBNP level both groups were lower than that before treatment, and the NT-proBNP level after treatment was(592.27±142.13) ng·L~(-1),which was lower than that of the control group(P<0.05).The scores of various heart failure symptoms of both groups after treatment were lower than those before treatment, and the scores of the study group were lower than those of the control group(P<0.05).Conclusion: Thumbtack needle combined with Blood Mansion Stasis-Expelling Decoction demonstrates definitive efficacy on patients with cardiac dysfunction in ischemic cardiomyopathy, can improve their clinical symptoms, cardiac function, biochemical indicators, and heart failure severity.

  • Action Mechanisms of Moxibustion in the Treatment of Knee Osteoarthritis

    The therapeutic mechanisms of moxibustion for knee osteoarthritis(KOA) primarily include: Anti-inflammatory effects through regulation of inflammatory cytokines; the mechanism of moxibustion in relieving knee joint pain(including neural regulation and inhibition of pain conduction);functional restoration mechanisms encompassing soft tissue repair and synovial fluid regulation.To advance moxibustion therapy for KOA,the following research priorities should be emphasized: conducting large-scale, rigorously designed clinical trials and mechanistic studies to comprehensively validate therapeutic efficacy; establishing unified treatment protocols and operational standards to enhance reproducibility and comparability of research outcomes; elucidating molecular and cellular mechanisms underlying its anti-inflammatory, analgesic, and regenerative effects; implementing multicenter randomized controlled trials to verify long-term clinical efficacy and safety; and adopting multimodal evaluation systems incorporating imaging and biomarkers for comprehensive therapeutic evaluation.

  • Research Progress of the Intervention of Steroid-induced Osteonecrosis of the Femoral Head by Traditional Chinese Medicine Through Regulating the PI3K/Akt Signaling Pathway

    The PI3K/AKT signaling pathway plays a significant role in the prevention and treatment of steroid-induced necrosis of the femoral head(SANFH).By activating a series of cellular signaling molecules, this pathway can effectively promote the generation of new bone tissue by osteoblasts, while simultaneously inhibit the activity and differentiation process of osteoclasts, thereby maintaining bone mass balance.At present, the commonly used single Chinese medicines for the clinical treatment of SANFH include Dasuan(Bulbus Allii),Gegen(Radix Puerariae Lobatae),Yinyanghuo(Herba Epimedii),Huangqi(Radix Scutellariae),Hongjingtian(Herba Rhodiolae),Gusuibu(Rhizoma Drynariae),Yinxingye(Folium Ginkgo),etc.The commonly used Chinese medical compound formulas include Himalayan Teasel Root Powder with Danggui(Radix Angelicae Sinensis),and Gusuibu(Rhizoma Drynariae),Gushi Formula, Necrosis of Femoral Head Rehabilitation Pills, Himalayan Teasel Root Pills for Supplementing Original Qi,etc.There have been relevant studies on the treatment of SANFH through regulating the PI3K/Akt signaling pathway with TCM,but the specific mechanism remains unclear and there are still many problems: Firstly, the mechanism and efficacy evaluation system of TCM in treating SANFH are not yet perfect, and there is a lack of sufficient scientific evidence to support it as the first-choice treatment.Secondly, due to the complex composition of Chinese medicinals, how to precisely quantify its role in the treatment process remains a major challenge.In addition, individualized treatment plans in TCM should also be adapted to the specific disease to better exert their therapeutic effects.In the future, it is necessary to continue to explore the advantages and potential of TCM in treating SANFH to provide new theoretical basis for the clinical prevention and treatment of SANFH.

    The etiology and pathogenesis of ovulatory disorder infertility(ODI) mainly revolve around kidney essence deficiency, phlegm-dampness obstruction, liver qi stagnation, qi stagnation and blood stasis, heart-kidney disharmony, and imbalance of yin and yang, which can cause disease independently or interact with each other.Kidney deficiency is the key factor causing ovulatory dysfunction.The kidney is the foundation of the body′s innate constitution.Only when the kidney qi is abundant can there be normal menstruation and ovulation.The treatment should focus on tonifying the kidney as the fundamental approach.While applying syndrome differentiation and treatment, it is also necessary to take into account methods such as soothing the liver, strengthening the spleen, harmonizing the heart and kidney, regulating qi and promoting blood circulation, and balancing yin and yang.Acupuncture and moxibustion for ODI improve the pregnancy rate by regulating endocrine, bettering ovarian function, influencing the quality of follicular development, and affecting the nerves that control the blood vessels around the follicles, etc.The relevant TCM therapies include acupuncture(such as flying needling, abdominal acupuncture, electroacupuncture, etc.),moxibustion, acupoint embedding, acupoint injection, auricular point pressing with seeds, and fire dragon cupping, and so on.However, clinical research lacks large-scale studies, and the experimental design is not rigorous and lacks rationality.Doctors′ selection of acupoints, acupuncture manipulation techniques, and treatment duration all inevitably involve personal experience and preferences.Therefore, according to the patient′s condition, individualized selection of acupoints through syndrome differentiation and treatment, and continuous optimization of acupoint combinations, acupuncture manipulation techniques, time duration, frequency, depth, retention time, and acupuncture tools to reduce the impact of individual experience differences among doctors on the therapies is the research direction in the future.
